Audiometric Testing
CAPABILITIES: Testing
utilizes mobile test vans equipped with individual testing booths. Testing vans
are also climate controlled to provide testing year round. Each of our testing
vans has the capability to test over 100 employees a day.
APPROACH: Our
approach to conducting this mobile testing service during normal production
operations does comply with OSHA 29 CFR 1910.95 and is outlined below:

Each employee will complete an "Ear
Health History" questionnaire form supplied by OHHCA.

Audiometric measurements would be
performed in accordance with OSHA regulation 29 CFR 1910.95. The following
additional specifications apply:
| a. |
Testing will be performed by trained
technicians who are under the supervision of an audiologist. Measurements are
made with calibrated equipment and in a controlled acoustical environment within
a mobile test facility which meets OSHA specifications. |
| b. |
Background sound levels will be
measured with an octave-band analyzer and documentation provided. |
| c. |
Testing is performed with a
recording audiometer at test frequencies of 500, 1K, 2K, 3K, 4K, and 6K (8K if
required) Hertz. |

The results of the testing will be
supplied in the following formats and will contain the following:
| a. |
A list in alphabetical order of all
personnel who received an audiometric testing, summary and analysis of all test
results for each employee by individual location. |
| b. |
A summary report identifying those
employees with standard threshold shifts, and OSHA 200 log recordable shifts by
individual location. |
| c. |
A computer generated notification
form which can be distributed to individual employees informing them in general
terms of the results of their audiometric measurements by individual location. | |
